I ran 2 scrims and seemed not to use my depth chart nor default OAI. I checked under scrimmages to make sure it was not being overridden there. What may cause that?

Only (and 1st) thing that comes to mind is, were they instant scrims? If so, then it's possible that your OAI was either not accepted in time or simply ignored. I seem to remember this issue before but it's been a LONG time back, for sure.

There's also an ancient glitch that may have not been fixed.
You have to auto reorder your depth charts before you actually set it. The theory is, buried on the depth chart are dots that are no longer on your team (ghost dots), which causes a glitch.
The auto reorder erases things that you can't see that are there.
