Does bullet vs loft come into play here? IRL, when we see those misses, it's usually because the pass was too flat to give the receiver time to run under it.

I would say this. If it's happening constantly, maybe something is a problem. If it happens occasionally, I would assume that's working as intended. No QB ever has been 100% accurate, especially on long passes

Well, I'm assuming you're not referring to the pass being slightly overthrown, but the receivers ability to catch it. Obviously Ballhawk VA doesn't affect the accuracy of the throw, but one would think it could help the WR catch the overthrown pass when it happens
Watch any NFL game and you'll see this happen. Sometimes a QB just overthrows a wide-open WR. It happens.
