When every receiver is covered so the QB holds the ball too long and even a 3-on-5 can't hold forever... especially with some of those SA's and VA's out there for D-linemen. (I should know... At the lower levels it's usually a vision issue. O-linemen just don't have enough to know how to spot who to block so, eventually, they're gonna blow the 'vision' roll and let somebody through.

At higher levels it's usually more about D-line SA's VA's and skills that cause the issue (that and DB's that can cover like a wet blanket).
