Do you just get them with level ups or do you have to trade something in? Flex?

They are free when you reach level 15, 20, and 25 (give or take 5 levels).

Yeah, and they will allow a slight increase to the cap of one ability each.

I think it depends on the attribute that powers it. Sometimes I'd add it to Conditioning and I'd get 4 points, sometimes I'd get 1.

If you try using it and you aren't happy with the result, you can hit cancel to get it back before you save the results.
