I just sent an offer to a player and it is not showing up in my team's "Sent Offers" page.

Just sent several more offers and those are showing up correctly. Might have been a one-time submission glitch but I'll keep monitoring.

yup. just had a player with a contract sent that doesn't show up in my players page also.

I had to send a few contracts twice to make them go as well

Possibly related to server lag.

Is the issue still occurring for anyone?

Had the same issue as soon as the site reopened and the site was pretty laggy at that time. I have not noticed contracts not going through since then.

Since we haven't gotten any more reports of this, I'm going to agree with VolBrian that it was most likely server lag.

NOT LIKELY A BUG

This is a sporadic issue that always seems to revolve around lag time, therefore, not likely a bug...verified.

I agree, this is lag not a bug, locking and moving.
