How exactly does this setting make a SS behave? I have a casual team and have tinkered with it a bit but not enough to get a good feel. How is a QB spy best used in a casual defensive AI?

It does the same thing as in a regular league. It will cause your SS to shadow the QB. After the snap he will move to parallel the QB and stay there until some magical moment when he decides it's time to tackle the QB.
