Well,
In every play, there's progressions.
As the QB goes through progressions, the sim is calculating a risk value. It's based on pass distance, closest defender and probably a few things we don't know.
The QB will throw the ball when the pressure of the defense pass rush exceeds the risk value (which is sometimes enough to piss off the Pope) or when a low risk target gets past the minimum distance for the play design.
The minimum distance is kind of unknown until you get really familiar with the passing plays. The new offense play creator allows you to choose the distance though.
The net effect of the favorite targets is to lower the risk of the target so the QB is more likely to throw the ball to them instead of continuing the progression.
