Is the only way to have your own team or become a GM to pay cash to the site or can you earn points somehow? Seems like a team that is ran by CPU should be able to be acquired?

Yup, you can coach or GM a team 100% for free if you can find an owner willing to let you do it.
In general, there aren't nearly enough good coaches/coordinators. There's always owners looking for folks that want to help out and take over some of the duties of running a team.

There are D League teams that have always been and always will be CPU teams. They are there to increase the number of available teams so (almost) everyone who wants to build a player can find a home.
CPU teams in non-D Leagues will be replaced in the offseason if league contraction occurs due to natural attrition. Teams above rookie cannot be purchased so contraction is the only way CPU teams disappear after rookie league.
