What goes into a team's ladder raking? Does last year's results factor in? Individual team ratings (passing, rushing, etc)? Just trying to figure it out - thanks!

The ladder ranking is an Elo-based rating that carries over from season to season (unless a team resets to Rookie, in which case it will be reset to the same rating as all other Rookie teams).
Each game you win, your rating goes up by some amount based on your rating and your opponent's rating. The bigger an underdog you are, the more your rating will increase; likewise, if you were a huge favorite, your rating will only increase a little. When you lose, your rating goes down similarly.
The only games that count are league games, playoff games, and ladder games. Scrimmages and tournaments (don't exist yet) do not.
