Yeah, if you are off-balance, you can't do anything very well.

I noticed huge issues on the test server with single man coverage almost always being blown, even when you had 85+ man awareness and pass coverage.

Not a real answer. The Ladder Games happen on the 3rd day of the game week, and match you up against someone roughly equal to your team's Ladder Ranking. League and Ladder games count toward the ladder rankings. It's a way to see how your team, and your league, match up against the other leagues.

"Ladder Cage Match" has a nice ring to it though.

This should be explained somewhere in the interface - maybe a mouseover tooltip when you hover over the blue and green bars?
