What do you currently have on the custom equipment and what are you trying to add?

i.e.

You have +3 to skills and you are trying to add +3 more
or
You have +6 to skills and are trying to add the SA tree

oh i see how the upgrades work now i thought i could add +1 skill points after level 8 but the upgrade is that you get the SA tree right?

Looks like you can't upgrade your custom equipment at lv16.

About the custom equipment, can you change the name or the picture later, or does it stay like that forever?
