OK, I was finally able to recreate the issue myself. I checked the page source, and the html looks exactly the same for the button that loaded incorrectly and the buttons that loaded correctly. So it looks like a bug with FF rendering the page inconsistently, though I'm sure it has nothing to do with the cache.

You probably just got unlucky enough to have it render wrong twice in a row.
