I am verifying this based on the three examples and the changelog..

LIKELY A BUG.

I think there may be a possibility that we aren't seeing what we think we are seeing. For instance like Bug03 pointed out that a head fake and juke are happening at the same time. Since there is no way for us to know that for sure, and the changelog says only 1 per tick, I am going to head and move this to be filed.
