Looking at the down, distance and location on the field, I believe the only way that would have happened was either by "preset" or Pikachus had their FG distance set at only 30 yards. They did attempt a 30 yarder in the game, and every 4th down in question that they went for it was between the 13.5 yard line (would be a 30.5 yard attempt) and the 30 yard line (47 yard attempt).
You can force yourself to go for it on 4th downs inside the 30 yard line like this by setting an unusually low FG distance setting. It would happen all game long though. There were no opportunities in this range prior to the 4th qtr so tough to say for sure.
