First step is nice for receiving HBs as they often have to run a bit further to get to their targeted area where the QB will actually pass to them. When I have used it, I have been less than impressed by first step as a power rusher. It is not bad for elusives though as it can fire after juke for a faster take off to push up field.
When I tried it on a power back my hope was to accelerate faster after breaking a tackle and being slowed down. It just did not seem to work that way though.

it does work well for counters. other than that it isn't that big of a help because it will fire on the start of the play but the handoff interaction slows him back down

Yup, counters and screens, basically if your speed back is also going to be a scat back then first step is worth it. You could make an argument for it on just a pure speedback for the benefit on counters alone.
