Could be a few things then. I obviously don't know what the receivers route tech/elusiveness is.
-It might be the low morale the CB had in each play.
-Could be too much cover tech and not enough footwork/quickness to support it. Seems as if the CB is ready to go in any damn direction, but he isn't able to go lol.

I think that may be what the issue is.

Happened on run plays, a play where my CB reacted fine and finished it off, as a CB4, and possibly at the end of one play.
The only time it didnt happen was when the receiver started with a slant or quick out...Definitely based on WR movement points.
