How do they come about- are they auto scheduled or does a team owner have to initiate ?

They are auto scheduled. The first one is on Wednesday and will be scheduled at rollover tonight.

Think about NCAA football for a minute. At the end of the regular season, there's always a big argument about "is a two loss SEC team better than a one loss PAC-10 team?" or whatever. The ladder was designed to solve that - every other game, you play against a team that is considered to be "similar" to yours, but isn't in the same league as you. You win, you move up the ladder - lose, you go down. At the end of the season, your team's ranking should then be a solid indicator of how you are. The best team will get the #1 ranking, second best #2, and so on.

Never was guaranteed to be within top 10. The code has always "preferred" teams within 10 ranks, but there have always been instances where this hasn't been the case.

This has always been especially true where ELO ties come in.

I'm pretty sure that there were early examples of something like 100 ranks difference playing.
