Looks like his agility is just too low to make the cut, but I'll mark this needs more info for further examples as per my PM.

Yea. Lack of agility/vision to see the end line and make a cut.

What's his agility/vision/quick cut?

I don't agree with how low #'s are working in the sim, but it's probably the reason. IMO no HB with a wide open view of the down field like that should go OB. I think it's an improvement that can be made, probably more for the suggestions forum. I've seen this similar thing happen at low levels numerous times.

He is a Level 16 playing in the minors. His Quick cut is ZERO.

He runs on 100% Power.
