i always assumed it went back to your basic tactics of run In/Out percentages. However with run out, i think its a 50/50 chance of getting either a Off T run or a pitch.

Run (random) randomly chooses between Outside and Inside runs based on your basic settings.
Run Outside randomly chooses between Off-Tackle and Pitch, with a 50/50 chance of each.
