With the qb being on -99 open man, why does the qb still stare at the WIDE open receiver and not pass? Is it a progression thing?

Probably. Got a specific replay? There's a bunch of different possibilities. The QB needs to feel pressured, or he'll wait for the best open man. The target needs to meet a specific goal, like being past the first down marker. Maybe your QB drop back speed vs progressions is out of whack. Maybe your QB is too blind to comprehend what he's looking at.

Plays have a built in "target depth" where the QB is hoping to see the target cross that depth before considering him open.
QB has to pass a vision check to notice the WR is in fact open.
QBs will favor situational football, meaning if its 3rd and 10 they will try to wait until the intended receiver is crossing the first down line.
Pressure on the QB negatively impacts their flow through progression and pass-throwing decision making.

All that being said, messing with the custom O play creator and tactics, I've set the preferences in 100 favor of target depth and set a deep depth on plays and the QB still randomly throws the ball 3 ticks after the snap for a 3 yard pass to a well covered receiver, without a "hurry" (though I'm fairly sure Bort has mentioned before a QB doesn't have to be "hurried" to be "pressured")

So in other words, don't expect a 100% realistic output that leans towards your tactics, its a simulation, that apparently goes down to the detail of like wind-resistance on passes (Recent Bort quote on discord I believe) so god knows why things are happening, bort himself probably couldn't tell you.

Lower it to somewhere between -50 to -70 and he'll get better. Also RR to between -45 to -65

One thing I didn't see mentioned but have always thought was a part of the process... which now that I can make O-plays I can SEE things I only thought about before and this knowledge was among that process... was that any RED route (primary receiver) had that happen more often than any other route. Especially if I target the red (primary) 1st. The QB tends to lock onto those receivers more often and longer. I'd have to have a couple of season's worth of replay stats to prove this but, as an OC and a scout, I've seen it for a long time and always wondered about it.
