It means he has spotted a blitz.

Edit to add..The QB can set his tactics to determine how he reacts to the blitz. Run the play as usual, Throw somewhat early, or throw early and often.

It might be in your settings how you view sim. I see it on both my pc and my smart phone.

nah, I checked them. It's set to show them. I see the I'm open things and catch fake, and power thru things in my game. So it may be because we're in different leagues
