Mine was working ... until I just looked at it.
Guessing it was using the local/cached version of the .js helper file in the require - and looking at it made it want the userscripts.org one. FMLife.

I have it on my comp at home but I am at work. I will try to remember to get it when I am at home
i dont know about any other versions but mine works fine, checked and it is 3.1.1 http://userscripts-mirror.org/scripts/show/113703
never got version 4.0.0 because this always worked fine -- remember to change the URLs in it

The 4.0.0 version just has the URL change done for you.

This doesn't load for me.
The install button stays greyed out and eventually I get a Download Error
