I saw this happen with my SS today. http://goallineblitz.com/game/replay.pl?game_id=2173338&pbp_id=2921479. I filed a bug and the response so far by a bug mod is "Fakes can fire without the overlay. It looks like a hard cut by the QB which we see a lot and a lack of tackle attempt by the SS. I dont believe it is the missed vision check but the hard cut. " Of course the hard cut doesn't explain the bounce effect that drives the SS back outside.
