Can I buy the permanent upgrades piecemeal? Can I buy a +2 speed equipment this season, then go back next season and add more speed to it or do I need to buy all 6 speed upgrades at once?

Yeah you can buy CEQ anytime, and upgrade it whenever you want to in the future.

Is it true that you get no refund if you spend reward points? I'm confused by the pricing of rented vs. permanent. Without a 70% refund it doesn't seem to make any sense to buy permanent equipment at 10 times the cost of rented....what am I missing?

Rented lasts 1 season, and no refund is given.
Permanent lasts the players entire career and gives a 70% refund of Flex Points

There is no refund given if reward points are used.
Buying Permanent CEQ is best for long-term players using flex.
Buying Rented CEQ is best for one-and-done players using reward points.

You only get back the flex you spend, so if you buy CE with 600 flex points and then upgrade it using 300 reward points, you'll get back 420 FP (70% of 600).

So you should use reward points if you choose to rent it and for the additional upgrades since they're not refundable anyway. The most you can ever get back from a refund of CE is 630 FP (70% of 900) if you use only flex points on it.
